Captopril - Drug Information
Indications Captopril is primarily indicated in conditions like Congestive heart failure, Congestive heart failure (adjunct), Diabetic nephropathy, Diabetic retinopathy, Essential hypertension, Essential hypertension (mild to moderate), severe hypertension resistant to other treatment, Gastric acid reduction during anaesthesia, Hypertension, Iron deficiency anaemia, Prophylaxis of acid aspiration, Renal hypertension, Scleroderma renal crisis
Dosage Adult: PO HTN Initial: 12.5 mg bid, 1st dose preferably at bedtime. Maintenance: 25-50 mg bid. Max: 50 mg tid. Heart failure Initial: 6.25-12.5 mg bid or tid. Maintenance: 25 mg bid or tid. Max: 50 mg tid. Post MI Start 3-16 days after MI. Initial: 6.25 mg/day, followed by 12.5 mg tid for 2 days, then 25 mg tid. Maintenance: 75-150 mg/day in 2 or 3 divided doses. Diabetic nephropathy Type 1 diabetics: 75-100 mg/day in divided doses.
Contra-Indications Angioedema related to previous ACE inhibitor treatment, hereditary or idiopathic angioneurotic oedema. Concomitant use w/ aliskiren in diabetic patients. Pregnancy.
Side Effects Renal Damage, Proteinuria, Hypotension, Respiratory Distress, Angioedema, Laryngeal Obstruction, Skin Rash, Taste Disturbance, Cough.
Special Precautions Patients w/ bilateral renal artery stenosis, collagen vascular disease, aortic or mitral valve stenosis, volume and/or Na depletion. Renal impairment. Lactation. Patient Counselling May impair ability to drive and operate machinery. Monitoring Parameters Monitor WBC count and urinary protein before and during therapy. Monitor renal function periodically.
Drug-Interaction Concurrent treatment w/ NSAIDs reduces hypotensive action and increases the risk of nephrotoxicity. Additive hyperkalaemic effect w/ K supplements, K-sparing diuretics, and other drugs (e.g. heparin). May increase risk of leucopenia w/ procainamide, allopurinol, cytostatic or immunosuppressants. May increase risk of lithium toxicity. Increased risk of nitritoid reactions w/ gold (Na aurothiomalate). Potentially Fatal: Increased risk of hypotension, hyperkalaemia, and changes in renal function (including acute renal failure) w/ aliskiren in diabetic patients.
