Indications Carvedilol is primarily indicated in conditions like Adjunct in heart failure, Angina,Angina pectoris, Cardiac arrhythmia, Cardiomyopathy, Congestive heart failure,Essential hypertension, Hypertension, Sedation for surgical and diagnostic proceduers,Sedation in intensive care, and can also be given in adjunctive therapy as an alternative drug of choice in Maintenance of anaesthesia, Myocardial infarction
Dosage Adult: PO HTN Initial: 12.5 mg once daily increased to 25 mg once daily after 2 days. May increase further at intervals of at least 2 wk to 50 mg once daily or in divided doses if necessary. Angina pectoris Initial: 12.5 mg bid increased to 25 mg bid after 2 days. Heart failure Initial: 3.125 mg bid, doubled to 6.25 mg bid after 2 wk if tolerated, then gradually increased to max dose the patient can tolerate at intervals of not <2 wk. Max: <85 kg: 25 mg bid; >85 kg: 50 mg bid. Left ventricular dysfunction post MI Initial: 6.25 mg bid, increase to 12.5 mg bid after 3-10 days if tolerated, and then to a target dose of 25 mg bid
Contra-Indications Bronchial asthma or related bronchospastic conditions. AV block, 2nd and 3rd degree. Sick sinus syndrome or severe bradycardia. Cardiogenic shock. NYHA class IV heart failure. COPD w/ bronchial obstruction. Metabolic acidosis. Severe peripheral arterial circulatory disturbances. Severe hepatic impairment.
Special Precautions Avoid abrupt withdrawal as it may precipitate thyroid storm, exacerbate angina, MI and ventricular arrhythmias. Patients w/ peripheral vascular disease, Prinzmetal"s angina, DM, untreated pheochromocytoma; worsening cardiac failure and fluid retention in patients w/ CHF during up-titration. May mask signs and symptoms of hyperthyroidism and acute hypoglycaemia. Pregnancy and lactation. Monitoring Parameters Monitor heart rate, BP, BUN, liver function
Drug-Interaction Decreased serum levels w/ rifampicin. Combination w/ Ca channel blockers (e.g. verapamil and diltiazem) can lead to bradycardia and myocardial depression. Potentiates insulin-induced hypoglycaemic action. May increase hypoglycaemic effects of antidiabetic agents. Increased risk of bradycardia w/ digoxin. Increased risk of hypotension and bradycardia w/ reserpine, MAOIs, clonidine. May increase ciclosporin concentrations. Concurrent use w/ ether, cyclopropane, trichloroethylene may increase the risk of hypotension and heart failure.
