Fluticasone - Drug Information
Typical usage: :asthma, chronic obstructive pulmonary disease
Probable Side Effects: :Glaucoma, Cataract, Growth retardation, Adrenocortical suppression, Decrease in bone mineral density.
Drug Interaction: Ritonavir (Norvir), atazanavir (Reyataz),clarithromycin (Biaxin), indinavir (Crixivan), itraconazole (Sporanox),ketoconazole, nelfinavir (Viracept), and telithromycin (Ketek) may increase levels of fluticasone in the body by reducing the breakdown of fluticasone byliver enzymes. This may increase side effects of fluticasone.
Mechanism of Action: Fluticasone utilises a fluorocarbothioate ester linkage at the C17 position. It has potent vasoconstrictive and anti-inflammatory activity, but weak HPA inhibitory effect when applied topically.
Pregnancy Impact: There is positive evidence of human fetal risk based on adverse reaction data from investigational or marketing experience or studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.
Impact During Lactation: There is positive evidence of risk to a breastfed infant or to breastmilk production, but the benefits of use in breastfeeding mothers may be acceptable despite the risk to the infant (e.g. if the drug is needed in a life-threatening situation or for a serious disease for which safer drugs cannot be used or are ineffective).
Formoterol - Drug Information
Typical usage: :Duodenal ulceration, for prophylaxis in pregnancy, prohylaxis of acid aspiration in obstetric patients.
Probable Side Effects: :Headache, palpitation, tachycardia, insomnia, tremor, peripheral dilatation.
Drug Interaction: Formoterol is known to interact withother drugs like quinidine, disopyramide, procainamide, phenothiazines, antihistamines, MAOI or TCAs, L-dopa, L-thyroxine, oxytocin, alcohol. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
Mechanism of Action: Formoterol stimulates the intracellular adenyl cyclase, the enzyme that catalyzes the conversion of adenosine triphosphate (ATP) to cyclic-3', 5'-adenosine monophosphate (cyclic AMP). Increased cyclic AMP levels cause relaxation of bronchial smooth muscle and inhibits the release of pro-inflammatory mast-cell mediators such as histamine and leukotrienes.
Pregnancy Impact: Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.
Impact During Lactation: There are no controlled studies in breastfeeding women, however the risk of untoward effects to a breastfed infant is possible; or, controlled studies show only minimal non-threatening adverse effects. Drugs should be given only if the potential benefit justifies the potential risk to the infant.
