Indications Acetazolamide is primarily indicated in conditions like Edema, Epilepsy, Fluid retention, Glucoma, Oedema, and can also be given in adjunctive therapy as an alternative drug of choice in Mountain sickness, Periodic paralysis
Dosage Adult: PO Diuresis 250-375 mg once daily or on alternate days. Glaucoma Conventional tab: 250-1000 mg/day, given in divided doses for amounts over 250 mg. Extended-release cap: 500 mg bid. Epilepsy 250-1000 mg/day in divided doses, either alone or w/ other antiepileptics. Prophylaxis of high altitude sickness 500-1000 mg/day in divided doses, preferably start dosing 24-48 hr before ascent and continue for at least 48 hr while at high altitude. IV Glaucoma 250-1000 mg/day, given in divided doses for amounts over 250 mg. Epilepsy 250-1000 mg/day in divided doses, either alone or w/ other antiepileptics. Diuresis 250-375 mg once daily or on alternate days.
Contra-Indications Hypersensitivity to acetazolamide, sulfonamides. Na and/or K depletion, suprarenal gland failure, hyperchloraemic acidosis, hepatic cirrhosis; chronic non-congestive angle-closure glaucoma. Marked hepatic and renal impairment.
Side Effects Blood Disorders, Skin Toxicity, Urinary Stone Formation, Aplastic Anemia, Stevens Johnson Syndrome, Potassium Loss, Metabolic Acidosis, Dizziness, Thirst, Headache, Drowsiness, Fatigue, Irritability, Excitement, Ataxia, Hyperpnea, Fever, Rashes, GI Upset
Special Precautions Patient w/ DM, resp acidosis. Elderly. Pregnancy and lactation. Patient Counselling This drug may cause some adverse reactions (e.g. drowsiness, paraesthesia), if affected do not drive or operate machinery. Monitoring Parameters Monitor intraocular pressure; serum electrolytes, periodic CBC w/ differential.
Drug-Interaction Concomitant use w/ high-dose aspirin may cause anorexia, tachypnoea, lethargy, metabolic acidosis, coma and death. May increase serum levels of phenytoin, resulting to increased risk of osteomalacia. May decrease serum concentrations of primidone. Additive effects w/ other carbonic anhydrase inhibitors. May increase the effects of folic acid antagonists. May decrease urinary excretion of amphetamine and quinidine. May prevent the urinary antiseptic effect of methenamine. May increase lithium excretion. Increased risk of renal calculus formation w/ Na bicarbonate. May increase ciclosporin levels.
