Phenytoin - Drug Information
Typical usage: :Convulsions, epilepsy, migraine prophylaxis, myotonia, prophylaxis of epilepsy, status epilepticus, status epilepticus; seizures in neurosurgery, TIC douloureaux, ventricular arrhythmias.
Probable Side Effects: :Ataxia, nausea, vomiting, hypotension, drowsiness, nystagmus, coma, hyperglycemia, stupor, blurred vision, skin rashes, elevated alkaline phosphatase, folate deficiency, hypocalcemia.
Drug Interaction: Phenytoin sodium is known to interact with other drugs like abacavir, abiraterone acetate, aceclofenac, acetazolamide (Na), acyclovir, alcohol, alcuronium (Cl), alfacalcidol, allopurinol, alprazolam, amiodarone (HCl), amitriptyline (HCl), amlodipine (Besylate), amoxapine, amprenavir, artesunate, aspirin, auranofin, azapropazone, bamifylline, beclomethasone (Dipropionate), benorylate, betamethasone, bleomycin, bromazepam, bupropion (HCl), busulphan, caffeine, calcifediol, carbamazepine, chloramphenicol, chlordiazepoxide (HCl), chlorpheniramine (Maleate), chlorpromazine (HCl), chlorpropamide, cholecalciferol, choline magnesium trisalicylate, cimetidine (HCl), ciprofloxacin, clarithromycin, clobazam, clofibrate, clomipramine (HCl), clonazepam, clopidogrel, clozapine, co-trimoxazole, corticotropin, cortisone (Acetate), cyclosporin A. Mechanism of Action: Phenytoin acts on sodium channels on the neuronal cell membrane, limiting the spread of seizure activity and reducing seizure propagation. By promoting sodium efflux from neurons, phenytoin tends to stabilize the threshold against hyperexcitability caused by excessive stimulation or environmental changes capable of reducing membrane sodium gradient. This includes the reduction of post-tetanic potentiation at synapses. Loss of post-tetanic potentiation prevents cortical seizure foci from detonating adjacent cortical areas.
Pregnancy Impact: There is positive evidence of human fetal risk based on adverse reaction data from investigational or marketing experience or studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.
Impact During Lactation: There is positive evidence of risk to a breastfed infant or to breastmilk production, but the benefits of use in breastfeeding mothers may be acceptable despite the risk to the infant (e.g. if the drug is needed in a life-threatening situation or for a serious disease for which safer drugs cannot be used or are ineffective).
