Torasemide - Drug Information
Typical usage: :Congestive heart failure, edema, hepatic disease, hypertension, renal diseases.
Probable Side Effects: :Hypotension, dehydration, hemoconcentration, hypokalemia, hypochloremic alkalosis, hypovolemia, hyponatremia.
Drug Interaction: Torasemide is known to interact with other drugs like amifostine, capecitabine, cholestyramine, colestipol (HCl), floxuridine, luconazole, fluorouracil, flurbiprofen, gemfibrozil, ibuprofen, indomethacin, ketoconazole, mefenamic acid, miconazole (Nitrate), niaprazine, perindopril, pirbuterol (Acetate), piroxicam, rituximab, sulfisoxazole, sulphadiazine, tolbutamide. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
Mechanism of Action: Torasemide inhibits the Na+/K+/2Cl--carrier system (via interference of the chloride binding site) in the lumen of the thick ascending portion of the loop of Henle, resulting in a decrease in reabsorption of sodium and chloride. This results in an increase in the rate of delivery of tubular fluid and electrolytes to the distal sites of hydrogen and potassium ion secretion, while plasma volume contraction increases aldosterone production. The increased delivery and high aldosterone levels promote sodium reabsorption at the distal tubules, and By increasing the delivery of sodium to the distal renal tubule, torasemide indirectly increases potassium excretion via the sodium-potassium exchange mechanism. Thus torasemide increases the urinary excretion of sodium, chloride, and water, but it does not significantly alter glomerular filtration rate, renal plasma flow, or acid-base balance.
Pregnancy Impact: Animal reproduction studies have failed to demonstrate a risk to the fetus and there are no adequate and well-controlled studies in pregnant women.
Impact During Lactation: There are no controlled studies in breastfeeding women, however the risk of untoward effects to a breastfed infant is possible; or, controlled studies show only minimal non-threatening adverse effects. Drugs should be given only if the potential benefit justifies the potential risk to the infant.
