Ibuprofen - Drug Information
Indications Ibuprofen is primarily indicated in conditions like Cystic fibrosis, Fever, Juvenile chronic arthritis, Migraine, Mild to moderate pain, Musculoskeletal pain, Nausea and vomiting (palliative care), Osteoarthritis, Pain, Post operative analgesia, Rheumatoid arthritis, Severe pain and tenderness in benign fibrocystic breast disease, Toothache, and can also be given in adjunctive therapy as an alternative drug of choice in Acute arthritis, Ankylosing spondylitis, Dysmenorrhoea, Inflammatory bowel disease.
Dosage Adult: PO Rheumatoid arthritis; Osteoarthritis 400-800 mg 3-4 times/day. Max: 3.2 g/day. Mild to moderate pain; Dysmenorrhoea; Fever 200-400 mg 4-6 hrly. Max: 1.2 g daily. IV Pain relief 400-800 mg 6 hrly, as needed. Max: 3.2 g/day. Fever Initially, 400 mg followed by 400 mg 4-6 hrly, or 100-200 mg 4 hrly. Max: 3.2 g/day. Topical/Cutaneous Pain and inflammation associated w/ musculoskeletal and joint disorders As 5% cream, foam, gel, spray soln or 10% gel: Apply as directed.
Contra-Indications Hypersensitivity. Treatment of perioperative pain in the setting of CABG surgery. Patient whom asthma, urticaria or other allergic-type reaction are precipitated by aspirin or other NSAIDs. Active or history of peptic ulcer; history of GI bleeding. Neonates w/ congenital heart disease, suspected necrotising enterocolitis, and impaired renal function. 3rd trimester of pregnancy, and lactation.
Side Effects Indigestion, Vomiting, Abdominal Pain, Heart Burn, Diarrhea, Dizziness, Skin Rashes, Sever Allergic Reactions. Heart Patients May Experience Swelling Of Body And Fluid Retention. Overdose May Cause Stomach Or Abdominal Pain And Ringing In The Ears
Special Precautions Bleeding disorders, CV disease. Infants w/ increased total bilirubin. Premature neonates w/ existing or at risk of infection. Hepatic and renal impairment. Elderly (avoid chronic use). Monitoring Parameters Monitor CBC, occult blood loss, hepatic and renal function.
Drug-Interaction Increased risk of GI bleeding w/ warfarin, corticosteroids, SSRIs and aspirin. May reduce the natriuretic effects of diuretics. Reduced antihypertensive effect of ACE inhibitors and angiotensin II receptor antagonists. May increase toxicity of lithium and methotrexate. Increased nephrotoxicity w/ ciclosporin and tacrolimus.
Paracetamol - Drug Information
Typical usage: :Ear pain, fever, headache, malaise, migraine, mild to moderate pain, pain, post-vaccine reaction, short-bowel syndrome.
Probable Side Effects: :Bronchospasm, blood dyscrasias, centribular necrosis, liver damage, hypoglycemic coma, hepatic necrosis, liver failure, skin rashes, GI adverse effects.
Drug Interaction: Paracetamol is known to interact with other drugs like alcohol, ascorbic acid, azilisartan medoxomil, busulphan, carbamazepine, chloramphenicol, cimetidine (HCl), diflunisal, interferon alpha, isoniazid, itopride (HCl), metoclopramide (HCl). Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
Mechanism of Action: Paracetamol is thought to act primarily in the CNS, increasing the pain threshold by inhibiting both isoforms of cyclooxygenase, COX-1, COX-2, and COX-3 enzymes involved in prostaglandin (PG) synthesis. Paracetamol indirectly blocks COX, and that this blockade is ineffective in the presence of peroxides. paracetamol is effective in the central nervous system and in endothelial cells but not in platelets and immune cells which have high levels of peroxides. The antipyretic properties of paracetamol are likely due to direct effects on the heat-regulating centres of the hypothalamus resulting in peripheral vasodilation, sweating and hence heat dissipation.
Pregnancy Impact: Animal reproduction studies have failed to demonstrate a risk to the fetus and there are no adequate and well-controlled studies in pregnant women.
Impact During Lactation: There are no controlled studies in breastfeeding women, however the risk of untoward effects to a breastfed infant is possible; or, controlled studies show only minimal non-threatening adverse effects. Drugs should be given only if the potential benefit justifies the potential risk to the infant.
