Indications Frusemide or Furosemide is primarily indicated in conditions like Congestive heart failure, Degradation of pancreatic enzyme suppliments, Edema, Heart failure,Hypertension, Long-term treatment of healed reflux oesophagitis, Oedema, Oliguria,Oliguria due to renal failure, Oliguria due to renal failure, Oliguria due to renal failure,Oliguria due to renal failure, Oliguria due to renal failure, Oliguria due to renal failure,Oliguria due to renal failure, Oliguria due to renal failure, Pulmonary edema, and can also be given in adjunctive therapy as an alternative drug of choice inBronchopulmonary dysplasia, Hemolytic-uremic syndrome, Hypercalcaemia, Raised intracranial pressure.
Dosage Adult: PO Oedema associated w/ heart failure Initial: 40 mg/day, may reduce to 20 mg/day or 40 mg on alternate days. In some cases, 80 mg or more daily in divided doses. HTN 40-80 mg/day, alone or in combination w/ other antihypertensives. IM/IV Oedema associated w/ heart failure 20-50 mg via IM or slow IV inj may increase by increments of 20 mg 2 hrly. Doses >50 mg must be given via slow IV infusion. Max: 1,500 mg/day. IV Acute pulmonary oedema 40 mg via slow inj over 1-2 min. If no adequate response w/in 1 hr, may increase to 80 mg via inj over 1-2 min.
Contra-Indications Frusemide or Furosemide is contraindicated in conditions like Anuria,Hepatic coma,Sulfone allergy,Hypersensitivity to the drug.
Special Precautions Frusemide should be used with caution in patients with any pre-existing liver or kidney disease, gout, diabetes, pancreatitis, lupus or allergy to sulfa drugs. This drug is not recommended for use during pregnancy or lactation. Furosemide causes photosensitization, so use sunscreen and wear protective clothing. Do not use furosemide in the late afternoon or evening. Consult with the doctor regarding dosing schedule. Do not porphyric patient.
Drug-Interaction Furosemide Is Known To Interact With Other Drugs Like Aliskiren, Amikacin (Sulphate), Arbekacin, Caffeine, Calcium Gluconate, Captopril, Carbamazepine, Cefaclor (Monohydrate), Cefazolin, Cefotaxime, Ceftizoxime (Na).
Typical usage: :All forms of epilepsy, Congestive heart failure, Electrolyte disorder, For prophylaxis in pregnancy, Hirsutism, Hyperaldoesteronism, Hypertension, Myoclonus, Nephrotic syndrome, Oedema, Oedema and ascites in cirrhosis of the liver, Parkinson's disease; drug-induced extrapyramidal symptoms, Primary hyperaldosteronism, Tobacco amblyopia and leber's optic atrophy.
Probable Side Effects: :Diarrhea, Drowsiness, Dizziness, Mental confusion.
Drug Interaction: Sodium excretion effect may be inhibited by aspirin. May reduce ulcer-healing properties of carbenoxolone.
Mechanism of Action: Spironolactone acts on the distal renal tubules as a competitive antagonist of aldosterone. It increases the excretion of sodium chloride and water while conserving potassium and hydrogen ions.
Pregnancy Impact: Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.
Impact During Lactation: Studies in breastfeeding mothers have demonstrated that there is significant and documented risk to the infant based on human experience, or it is a medication that has a high risk of causing significant damage to an infant. The risk of using the drug in breastfeeding women clearly outweighs any possible benefit from breastfeeding. The drug is contraindicated in women who are breastfeeding an infant.
