Metronidazole - Drug Information
Typical usage: :Amoebiasis, giardiasis, trichomonas vaginitis, anaerobic infections, ulcerative gingivitis. Eradication of H.pylori associated with peptic ulcer disease. Antibiotic-associated coulitis. Trichomoniasis, Balantidiasis, Blastocyatis hominis infection. Adjunct to treatment of chronic periodontal infections. Rosacea.
Probable Side Effects: :GI disturbances e.g. nausea, unpleasant metallic taste, vomiting, diarrhoea or constipation. Furred tongue, glossitis, and stomatitis due to overgrowth of Candida. Rarely, antibiotic-associated colitis. Weakness, dizziness, ataxia, headache, drowsiness, insomnia, changes in mood or mental state. Numbness or tingling in the extremities, epileptiform seizures (high doses or prolonged treatment). Transient leucopenia and thrombocytopenia. Hypersensitivity reactions. Anaphylaxis.
Drug Interaction: Acute psychoses or confusion with disulfiram. It has synergistic effect with other antimicrobials. Effects reduced with phenobarbital or phenytoin. Disulfiram-like reaction with alcohol. Increased risk of adverse effects of coumarin anticoagulants, phenytoin, lithium, ciclosporin, fluorouracil. Increased risk of neurological effects with cimetidine.
Mechanism of Action: Metronidazole is converted to reduction products that interact with DNA to cause destruction of helical DNA structure and strand leading to a protein synthesis inhibition and cell death in susceptible organisms. It is effective against a wide range of organisms including E. histolytica, T. vaginalis, Giardia, anaerobes e.g. Bacterioides sp, Fusobacterium sp, Clostridium sp.
Pregnancy Impact: Animal reproduction studies have failed to demonstrate a risk to the fetus and there are no adequate and well-controlled studies in pregnant women.
Impact During Lactation: Drug which has been studied in a limited number of breastfeeding women without an increase in adverse effects in the infant; And/or, the evidence of a demonstrated risk which is likely to follow use of this medication in a breastfeeding woman is remote.
Norfloxacin - Drug Information
Typical usage: :Uro-genital infections. Typhoid fever. Acute bacterial gastroenteritis. Chronic bacterial prostatitis. Gonorrhoea, Prophylaxis of sepsis in neutropenic patients. Eye and ear infections.
Probable Side Effects: :Hepatitis, hemolytic anemia, seizures, exfoliative dermatitis, interstitial nephritis, headache, nausea, vomiting, diarrhea, fever, constipation, insomnia, dry mouth, rashes, GI symptoms, depression, arthralgia.
Drug Interaction: Norfloxacin is known to interact with other drugs like aluminium hydroxide and oxide, caffeine, cyclosporin A, probenecid, theophylline, warfarin (Na), Zinc Sulphate.
Mechanism of Action: Norfloxacin inhibits the enzymes topoisomerase II (DNA gyrase) and topoisomerase IV, which are required for bacterial DNA replication, transcription, repair, and recombination. Norfloxacin is a broad-spectrum antibiotic that is active against both gram-positive and gram-negative bacterias. The fluorine atom at the 6 position increases potency against gram-negative organisms, and the piperazine moiety at the 7 position is responsible for anti-pseudomonal activity
Pregnancy Impact: Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.
Impact During Lactation: Studies in breastfeeding mothers have demonstrated that there is significant and documented risk to the infant based on human experience, or it is a medication that has a high risk of causing significant damage to an infant. The risk of using the drug in breastfeeding women clearly outweighs any possible benefit from breastfeeding. The drug is contraindicated in women who are breastfeeding an infant.
