Metronidazole - Drug Information
Typical usage: :Amoebiasis, giardiasis, trichomonas vaginitis, anaerobic infections, ulcerative gingivitis. Eradication of H.pylori associated with peptic ulcer disease. Antibiotic-associated coulitis. Trichomoniasis, Balantidiasis, Blastocyatis hominis infection. Adjunct to treatment of chronic periodontal infections. Rosacea.
Probable Side Effects: :GI disturbances e.g. nausea, unpleasant metallic taste, vomiting, diarrhoea or constipation. Furred tongue, glossitis, and stomatitis due to overgrowth of Candida. Rarely, antibiotic-associated colitis. Weakness, dizziness, ataxia, headache, drowsiness, insomnia, changes in mood or mental state. Numbness or tingling in the extremities, epileptiform seizures (high doses or prolonged treatment). Transient leucopenia and thrombocytopenia. Hypersensitivity reactions. Anaphylaxis.
Drug Interaction: Acute psychoses or confusion with disulfiram. It has synergistic effect with other antimicrobials. Effects reduced with phenobarbital or phenytoin. Disulfiram-like reaction with alcohol. Increased risk of adverse effects of coumarin anticoagulants, phenytoin, lithium, ciclosporin, fluorouracil. Increased risk of neurological effects with cimetidine.
Mechanism of Action: Metronidazole is converted to reduction products that interact with DNA to cause destruction of helical DNA structure and strand leading to a protein synthesis inhibition and cell death in susceptible organisms. It is effective against a wide range of organisms including E. histolytica, T. vaginalis, Giardia, anaerobes e.g. Bacterioides sp, Fusobacterium sp, Clostridium sp.
Pregnancy Impact: Animal reproduction studies have failed to demonstrate a risk to the fetus and there are no adequate and well-controlled studies in pregnant women.
Impact During Lactation: Drug which has been studied in a limited number of breastfeeding women without an increase in adverse effects in the infant; And/or, the evidence of a demonstrated risk which is likely to follow use of this medication in a breastfeeding woman is remote.
Ofloxacin - Drug Information
Typical usage: :Genito urinary, respiratory, gastro intestinal. skin and soft tissue infections. Peritonitis, Gonorrhoea. Typhoid fever, Multi drug resistant tuberculosis. Acute and sub-acute eye and ear infections.
Probable Side Effects: :Sinus tachycardia, hallucinations, stevens johnson syndrome, seizures, tendon rupture, dizziness, headache, nausea, vomiting, diarrhea, insomnia, abdominal pain, pruritus, photosensitivity.
Drug Interaction: Ofloxacin is known to interact with other drugs like aluminium hydroxide and oxide, didanosine, iron Salts, pentostatin, probenecid, procainamide (HCl), sucralfate, theophylline. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
Mechanism of Action: Ofloxacin acts on DNA gyrase and toposiomerase IV, enzymes which, like human topoisomerase, prevents the excessive supercoiling of DNA during replication or transcription. By inhibiting their function, the drug thereby inhibits normal cell division.
Pregnancy Impact: Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.
Impact During Lactation: Studies in breastfeeding mothers have demonstrated that there is significant and documented risk to the infant based on human experience, or it is a medication that has a high risk of causing significant damage to an infant. The risk of using the drug in breastfeeding women clearly outweighs any possible benefit from breastfeeding. The drug is contraindicated in women who are breastfeeding an infant.
